Self-Managed Packets
Transcript Requirements
Transcripts are required from all colleges or universities:
- from which the applicant received a degree;
- where the applicant was enrolled in a degree program for more than a single semester.
- In addition, transcripts should be provided from all institutions where the applicant was enrolled in coursework relevant to the degree program identified in this application. This may include courses taken in non-degree status, in transient status, or in post-baccalaureate status, and is regardless of whether or not the courses led to a degree. (If in doubt as to the relevance of a transcript, go ahead and arrange for it to be sent.)
A transcript showing the completion of any undergraduate degree program currently in progress must be on file before registration for classes.
Not required are:
- Transcripts from schools for AP courses the applicant took in high school.
- Transcripts from schools when the student was enrolled in a summer abroad program, a summer internship program, or other enrichment activities that did not involve the applicant being enrolled in a degree program, unless that work is relevant to the program identified in the current application. (See above).
GRE Scores
You are required to include in your packet photocopies of your official score report of the Graduate Record Examination, if it is required by your department for admission. Not including these scores could delay the processing of your application. Applicants are advised to take this examination early in the application process to allow for unexpected delays in records management.
Please use the Georgia State University institutional code 5251 to ensure that your scores are sent to Georgia State University. Scores on the Graduate Record Examination are downloaded periodically into the Georgia State Student Records System. This acquisition is made at the University level and can take various periods of time, as long as six weeks. Every effort is made to apply the scores to student records in a timely manner, but no guaranteed time is possible. Official Scores must be on file before an application is transferred to the department as "complete."
Letters of Recommendation
Collect letters from your recommenders in sealed envelopes; recommenders must sign their names across the sealed flap of the envelope. Submit the letters unopened with the rest of your application packet. Be sure to include the Letter of Recommendation Waiver Form to instruct us whether or not you are waiving your privilege to review these letters on our premises at a later date. The letters cannot be open or in unsealed envelopes or folders. Some departments do not ask for written letters, but request a List of References. If this is a requirement for your application, include this form in your packet.
Other Supporting Documents
Portfolios, writing samples, publications and other evidence of achievement must be clearly identified in the packet and in the format prescribed by the department concerned. This information can be obtained by reviewing the department Web site and by contacting the graduate director for that department before submitting your application materials to the Graduate Office.
Certificate of Immunization
The Board of Regents of the University System of Georgia requires that accepted applicants who have never attended Georgia State University submit proof of immunization from measles, mumps and rubella before being allowed to register. A form for providing this certification to the Georgia State University Health Services is available on the Worldwide Web site. Questions about the requirement may be directed to the immunization unit (147 Sparks Hall, Phone: 404/651-1171; Fax: 404/651-1238). Immunization information must be on file with the university prior to an accepted applicant's first registration. Send the certificate directly to the Health Services Office.
